Case Study

35 y/o white female presents to the ED after a motor vehicle accident 3 days prior.  The MVC was a rear end collision while she was at a stop light.  Mild to moderate damage to her car was noted; she was wearing a seat belt, no air bag deployed.  She states she was seen in an ED in the neighboring town after the accident and had xrays of her chest, right hip, and a CT scan of her neck which all were “normal.”  She was Rx oxycodone-acetaminophen 5/325 mg (Percocet), Methacarbamol (Robaxin) 750 mg, and Naproxen sodium (Naprosyn) 500 mg.  She has been taking the meds as Rx but is “no better.”  She is requesting a refill of the oxycodone because “that is the only one doing anything” for her pain.  
In your response, answer:

1.  Develop a plan of care for this patient–best practice, you may include pharma and non-pharma options.
2.  What is the role of opiates in the care of this patient with acute pain?  Yes, no??
3.  What are some “red flags” we as providers should be tuned into when concerned about opioid use/abuse?
